Enter a New Patient Appointment
A new patient calls for an appointment. Go to the 2nd blue tab, appointment book
Select my calendar in the top left navigation panel
Select the doctor or therapist in the top left navigation panel
Click on the day you want to schedule the appointment moving through the appointment book with the arrows in the upper right and left corner of the calendar screen.
Click on the time you want to schedule the appointment by double clicking in the blank space in the calendar.
click the new patient button on the bottom of the ‘Add Event’ pop up box
Enter first name a mandatory field: John
Enter last name a mandatory field: Adams
Enter an Email address a mandatory field for the patient portal: john adams at gmail.com
click save
Select purpose of visit: New Encounter
Check the appointment time to make sure it is correct and click save
observe a new appointment has been added to the screen
Note: Let the patient know that an activation email will be sent to their email address with a link to create a password for their patient portal

A New Patient Arrives for Their Appointment

Go to the 2nd blue tab, appointment book
Select my calendar in the top left navigation panel
Select the doctor or therapist in the top left navigation panel
Find patient and double click to open the appointment
Change the status from scheduled to checked in
click Save
right click on the appointment
the go to patient option will pop up to the right
Double Click go to patient
Click Edit to bring up the patient profile pop up box
Check that each field has been completed, if not give the patients forms to fill out or a tablet to enter their information on
Click the second green tab in the patient profile to confirm insurance
Click the third green tab in the patient profile to select which module to share with patient
Click save to close the profile editor
take a picture of the patient with digital camera connected to the computer to upload into DocuRehab, see uploading patient picture for details
Collect patient documents for upload into the system these will go under the third green tab storage, see scanning documents for details

To Upload a Picture to a Patient Account

Click change picture from the gray menu
Observe the upload picture window pops up
Click Browse
Observe a window explorer window pops up
Click to select a picture
click open
Move the rectangle and increase or decrease the size to focus on the face
click Save
Observe the patient picture has been uploaded.

To Upload Scanned documents to Patient Account

to access the Storage Bin where the documents will be kept by clicking on Storage, which is the third tab on the Patient page.
Then click on”New” and a pop up box labeled “Add Storage” will open.
Click “Upload File”.
Select the file you scanned, we will choose a digitized x ray for this. Click Open.
You will then see the filename and extension for the file that you selected.  On the right you will put the name of the file that you want to be seen in the Storage Bin.  We will call it a “cervical”
In the drop down menu, you will see a series of categories for where you want to place the file within the Storage Bin.  
We will choose digitized x ray ” then “Save.”
Automatically you will see a images of x rays and the word Cervical  

Doctor Preview of Patient Information

Go to the 2nd blue tab, appointment book
Select my calendar in the top left navigation panel
Select the doctor or therapist in the top left navigation panel
Find patient and right click on the appointment
the go to patient option will pop up to the right
Double Click go to patient
If the patient has activated their patient portal a green tab name patient portal will be the last green tab underneath the patient information
Click the patient portal tab
Click play to load the information into the player for review
Click next in the top right corner of the player screen to move from one form to the next.
When all charts have been review click close, observe the patient medical history
Click the third green tab, storage
Each type of document is indicated by the picture in the bottom left of the screen
Double click on the icon, observe a cervical xray opens in a new window
A new browser tab will open with a PDF image of the document.
Multiple Documents can be opened at one time, each indicated by a new browser window to review at the same time, observe another window opens with the motor vehicle accident report
Close additional browser times and the Dr or therapist is ready to see the patient

Entering a New Patient Initial Visit

From the appointment book right click on the patient name and click go to patient
Click the green tab service in the middle of the screen
Click on new in the gray menu just below this green tab
observe a patient’s player appears at the bottom of the screen named: new service for john adams
Click the screen top blue tab: Protocols
In the left navigation pane Click charting
In the left navigation pane  Click Doctor portal
Click new patient evaluation
A green icon call Automated patient evaluation will populate the grey area of the screen
Click the automated patient evaluation icon and add to service in the top toolbar OR click and drag icon into patient player at bottom.
Click save and play in the bottom gray menu of the patient’s player
observe now: there are four icons in the left navigation pane 1 , 2 , 3 , 4
Each of these Icons represent a section of the patient visit
Summarize Risk Factors from new patient paperwork this patient has a pacemaker and cardiovascular disease, click next
Summarize Complaint history from new patient paperwork, this patient has been in a work accident and fell on their side- symptom neck sore- past treatment antiinflammatory by md, click next
Click the box that corresponds with the body area of the complaint, we will choose neck
See on the left a new Icon has been pulled in, SOAP ½ NECK, this is the subjective and objective part of your notes
Fill in the Complaint this is the only complaint so it is  #1
The type is pane
the location is neck left side
The progress is worst
Radiating to the right of the shoulder
the quality is acute
the intensity is rated 7 out of 10
asymmetry listing at C5 and C6
palpation reveals tenderness of neck and right shoulder
And the spasm of right upper trapezius
First orthopedic tests we will select foraminal compression
Observe a new icon is added in the left navigation pane named foraminal compression
We will also select a neck range of ROM test by clicking the box, observe another new icon is added to the left navigation
Please note all items in green font are linked to other modules, we refer to as anchored Click next and the Neck ROM will be the next screen, active flexion 80, active right rotation 75 and active left rotation 60, click next
The Foraminal compression test, which will be positive, click next
The next module labeled SOAP 2/2  is the Assessment and plan
This is the acute phase
Patient work status is off work
Activity of daily living is moderately affected
The patient has not been referred out, leaving  NA in the option box
This will prevent the entire question from printing in your notes, maintaining only the pertinent information to the individual patient case.
short term goal 1 is pain will decrease
short term goal 2 is range of motion will increase
click close and your initial evaluation is complete for this patient
Observe the charting module icon are now displayed as an animation in the bottom right of the screen, while the date of service is posted and selected immediately to the left.

Selecting a Diagnostic

Before the billing can address the patient service the doctor or therapist must select the diagnostic codes
Click top left blue tab: patients
Click in the left vertical screen panel in the top search field and enter the patient name: john Adams
observe john adams is selected and appears in the top right side of the screen
Click the green tab diagnostic in the middle of the screen
click on manage diagnostic
observe the pop up window
There are 2 ways to establish the diagnostic
first way to proceed as an example: go to diseases of the musculoskeletal system----dorsopathie...spondylosisis and down up to what you are looking for or the second way
enter a code or a name in the top search box: example lumbar
observe that every ICD9 codes pertaining to the lumbars will pop up.
Select the code 722.10 and drag it on the right side of the pane.
To enter a second code select 724.02 and drag in in the right side of the pane
observe that the top diagnostic is the primary diagnostic,
To change the primary diagnostic select the bottom code 724.02 and click on the arrow “move up”
observe the primary diagnostic is now 724.02
To remove a diagnostic just click on the red “x” on the right.
Note that all past diagnostic are recorded inclusive of their ranking at a specific date

Return Patient Service/SALT

From your Appointment book, right click on patient and click go to patient
john Adams
observe john adams is selected and appears  in the top right side of the screen
Click the green tab service in the middle of the screen
Click on the last date of service, again click the last date of service for this patient
Click SALT
SALT stands for Same As Last Time, this will duplicate the prior appointments modules into a current player
observe:a new Patient’s player appears in the bottom and contains the exact same icons as during last visit
Click Play
Observe that the first chart module displays the patient’s data as entered in the last visit
You will modify this data slightly according to the evolution of the patient’s condition.
The Risk and Complaint history will not change
They may or may not have a new complaint to indicate, other wise click next until you come to the module that says SOAP ½, these are the factors that may change from one visit to another

observe: Typically only 4 to 5 elements need to be changed in the whole record keeping your documentation well  below the minute mark.
You will not repeat the tests in these follow up visits, unclick the boxes anchored to the orthopedic tests, observe the icons for those tests have been removed from the left navigation
click next
click next
click next then close,
your return patient’s encounter is fully documented.
Note: A typical initial evaluation will have a higher number chart modules than a regular visit as the initial exam is more thorough.

Adding Treatment to A Patient Service

Click the top left blue tab: patient
select john adams.
You have just created a new charting for john adams and you are ready to treat him
click on the green center service tab
and click edit from the grey menu
observe that in the bottom of the screen, the patient player appears named: New service for john Adams
In the patient player gray menu observe the date of service window
The date of service window naturally default to today’s date but an earlier date can be picked if you had missed an entry.
click  the top blue tab named protocol,
in the left pane
select public protocol..........  Treatment,  Treatment by Area,
for this example we will select NECK
Double click on the neck
in the right pane
animate the icon by selecting in the top grey menu ….......View..........animation
Roll over successively on different animation leaving the cursor a few seconds, observe that the billing code embedded in this animation appears as a cue tip.
Click on several of those animation and drag them down in the patient’s player
or click on one animation to select it
and then click on the button  add to bottom
once you have populated your player with different treatments
click save and play.
the player is now open
the treatment icons are now displayed in  the left pane
Click on the treatment icon on the left pane directly
by clicking any icons on the left pane you are playing the animation in the right pane
observe the green tab in the bottom named option; this tab allows you to document the treatment for a specific patient by entering day to day variables.
If you click variable you will see the billing codes associated with the animation as well as description of the animation as you roll over
you navigate through all the treatment by clicking on the button Next from the top gray menu
Click close
observe that the animated treatment are playing as animated icon while the date of service is selected;
to turn the animation off,
click view...........thumbnail
observe the animation has disappeared

Changing Treatments

Click the top left blue tab: patient
select john adams.
click the date of the last service
Click SALT to duplicate the patient’s last visit
To change the treatment listed in the player click the icon of the treatment
click remove in the toolbar above the player
Repeat this for additional treatments you wish to remove
To add new treatment, go to the blue tab Hands On
Click Public Hands on
select the category of the treatment you wish to perform
We will select HVLA
Click HVLA cervical
Click on several of those animation and drag them down in the patient’s player
or click on one animation to select it
and then click on the button  add to service
once you have populated your player with different treatments
click save and play your treatment service has been edited for the current visit

Re-evaluation for Patient Progression

Click the top blue tab, patients
Select John Adams in the left pane
Observe that John Adams has 3 visits
The first date of service is the listed at the bottom and
The most recent visit is at the top
Click the last date of service on the top
Note to the right there are five icons occupying the screen
Click the first date of service
Note to the right there are eight icons occupying the screen representing the initial evaluation including orthopedic tests
To re-evaluate the patient for progression click the date of initial service at the bottom and click SALT
The player has been populated with the same icons as used during the initial evaluation
Click play
The data added from the first visit is saved in each screen.
The Risk factors will remain the same, click next
The complaint history will remain the same, click next
The area of complaint too remains the same, click next
On the SOAP ½ chart Make changes to the pertinent fields to represent the patient’s current condition
We will change the progress from worse to better
The quality is now dull
The intensity has changed to a 2 from a 7
Click next and perform the same orthopedic tests, this was ROM for neck
Click next
We will check the short term goal pain has decreased
Click close
Your update is done using the same data as during the precedent exam for easy comparison
You can compare side by side on the same page two modules by clicking the module SOAP neck ½ and then click compare history.
Observe the data are side by side

Using a Protocol

Click on the top blue tab, patients
Select a patient, we will select John Adams
Click new in the toolbar below the green service tab
Click the top blue tab, protocol
Note: A protocol is used to group content material for quick retrieval
In the left vertical pane click ‘public protocol’
Click treatment
Click by condition
Note several green icons appears in the grey right panel, click spondylolisthesis lumbar
Click the spondylolisthesis lumbar icon and drag it into the patient service bar
Observe four common  treatments for spondylolisthesis lumbar now populate the service bar
Click save and play
The player is now populated with four treatments allowing you to move from one to the other by clicking the next button in the top right of the grey panel
In the public section of DocuRehab, we have grouped the icons in charting module and in treatment for ease of use. you can organize them as you see fit in “my protocols” section

Organizing MY Modules

Overview of Public vs. Personal Database Areas
DocuRehab modules for charting, hands-on, exercise and modalities have two parts: the public database and the users’ personal database. The personal database is indicated with the work ‘My’ prior to the module type example My charting.
The public database is populated for all users to draw from and may be added by a user when they have created a new chart, treatment, exercise or modality and it has been issued for publishing.
The personal database is where the user may customize the modules to create and represent specifically what they do. The personal database is exactly that, personal. Other users are not able to draw from one another unless modules are shared or published.
To maintain order in the personal database, DocuRehab strongly encourages users to create folders in their ‘My’ areas prior to editing or creating to ensure organization and ease of use.
We will use charting as an example of how to make a new folder in ‘My Charting’ for use later when we edit and create new charts.

Organizing ‘My Charting’

DocuRehab enables you to edit current charts and create new charts in the ‘my charting’ section
Before you start to edit and create your charts it is important to set up folders in ‘my charting’
Click the top blue charting tab
click my charting in the left navigation pane
click the black arrow adjacent to the new button in the toolbar above the right grey panel
click Category
A pop up window comes up asking you to name this folder
We will mimic the public and use Subjective and hit enter
Observe a folder now is under the My charting region of the left navigation panel.

To Edit a Protocol

Click the blue tab: protocol
In the left pane click public protocol
Select the type of protocol to edit: charting or treatment
Select treatment in the left vertical pane
Select By condition  in the left pane
We will edit the protocol for Spondylolisthesis
Right click on spondylolisthesis in the right vertical pane and click copy
Click ‘My Protocols’ in the left vertical pane
Right click in the grey area to the right of the vertical pane and click paste
Observe the protocol is now listed in the left pane
In the grey area to the right,  Click on spondylolisthesis
Click on  Edit in the toolbar directly above
The protocol bar at the bottom appears populated with Icons representing different treatment associated with the protocol
Click on the treatment  you wish to remove the lumbar drop to select it
and click ‘Remove’ from the toolbar directly above
Note the icon has now disappeared
Items may be added to a protocol from any of the modules: Hands on, exercise, modalities and charting
Select the appropriate blue tab on the top, we will add a hands on treatment to this protocol
Click the blue tab hands-on
Click public hands on in the left pane
Click LVLA (low velocity, high amplitude)
Click Patient passive, Dr active with instrumentation
Click the treatment icon in the grey panel
Note the ‘Add to protocol’ button on the top toolbar is now highlighted
Click add to protocol
Note the treatment icon is now in the protocol bar
Click save, the protocol has been edited and will be available for patient services in the My protocol section

Billing without Icons

DocuRehab can generate a superbill in a traditional manner as well as with icons with assigned billing codes
To create a bill from the SOAP notes through the patient tab without icons
Click the Blue patient tab
Select the patient we will generate a bill for: John Adams
Click on the green billing tab
Click on the date of the service we are creating a bill for
Every part of the patient service is in the left column under treatment name on a single row
Each row displays N/A as the default
N/A will not be printed in the bill
We have selected an initial evaluation without treatment
We will change the CPT code from NA to new patient evaluation
Double click on N/A
Observe a scrolling list of most common CPT codes appears and when the mouse rolls over the code a description of the code appears in a tooltip, scroll to select the right code
This will be a physical therapy evaluation, Click 97001
Or after double clicking the NA
Delete the NA
Type 97001 and hit enter
Next we will create a superbill for a patient who had chiropractic treatment
In the left navigation listing all the patients select the patient we are going to create a bill for, we will choose Sarah Lopez
Click the green billing tab
Click on the service date to select it for superbill generation
Click new on the toolbar above
Observe a new row appear
Click on the “billing code” column of that new row
A drop down menu with a list of CPT codes appear
Scroll down the list of codes for the correct one
Select the correct code by clicking on it, we will select 98940 for treatment of 1 to 2 regions
Observe the code description is now posted in the “treatment name column”
To edit the name of the treatment
Double click on the treatment column row
Delete one to two regions, type one region cervical and hit enter
Observe the treatment name has changed
To edit the date double click the date under treatment name and adjust to be correct
Click Print
The system will print a billing report or to a 1500 claim form or  HICFA form for those services which have had a code and not N/A

Billing with Icons

The primary advantage of billing with icons is each treatment icons represent a service rendered to the patient for billing. This creates a quick reference to ensure you do not over bill, under bill or bill for the incorrect service
Click the blue tab: patients
Select the patient we will bill: we will select John Adams
Click the date of service we will be billing
Observe in the bottom right panel the icons represent different parts of the patient service and when you slowly roll over the treatment icons the CPT code attached listed on a tooltip
Observe some treatment icons have multiple CPT codes separated by a comma displayed
This means that the icon can be used to generate any of those CPT codes and
The user will have to determine the correct CPT code based on the entire patient service
If the treatment is time sensitive in terms of: it must be completed for x amount of time in order to be paid by insurance or medicare/medicaid, the treatment has had an area to enter the time for the treatment
Click the green billing tab
Click the date of service we are billing
On this date there are two exercises with the same billing code but only one was completed to requirement for payment
These rows will need to be merged to create a billable exercise
Click the first exercise and then hold the control key down and click the next exercise
Observe two rows are now highlighted
Click the merge button in the toolbar above
Note the two rows are now one with the CPT code listed
If two rows are merged with more than one CPT code listed, double click on the code and select the correct one

Modifying a Bill

To modify a bill select the date of service under the green billing tab by clicking on it to expand the services associated with this bill
Determine the treatment or exercise we will add a modifier for
Double click in the column modifier
If you are not sure of the correct modifier: roll over the modifier and let the cursor sit to allow the tooltip to appear with a small description of the modifier
To delete a row
Select it and click delete in the toolbar above
Observe that the row has been crossed out by a red line but has not been removed in the service
Click on the service tab
Observe that the treatment deleted in billing is still associated in the service
To add a treatment which is not represented already
Just select the date click new
A New treatment row will appear
Double click N/A to select or type in a code
IF a mistake occurs and you want to start all over from the service
Click reset and everything is returned to its’ original state

Editing Hands On Treatments

Click the top blue tab, Hands-On
In left vertical pane click ‘public hands on’
Choose the type of treatment: HVLA (high velocity, low amplitude), LVHA (low velocity, high amplitude), LVLA (low velocity, low amplitude)
We will select HVLA
Select the area of the body the treatment is for ex. cervical, lumbar, extremities
We will select Cervical HVLA by clicking on it in the left vertical pane
Observe the grey pane to the right is now populated with Treatment Icons
Right click on the treatment Icon you wish to edit and click copy
We will choose Sitting Cervical transverse AP move
In the left vertical pane click ‘My Hands On’
In the grey center pane, right click and click paste
Note the  Sitting Cervical transverse AP move icon is now listed in the grey screen
Click the Sitting Cervical transverse AP move Icon and in the toolbar above click Edit
The Hands-On wizard is now open
Note to the left are 11 buttons, each accounts for a different part of the treatment
We will change the difficulty of this from medium to easy by clicking in the bottom right grey option area, note difficulty is determined by clicking a radio button signifying the level of this particular treatment
Note boxes in red in the top left portion of the screen directly relate to the items in the bottom right panel
The wizard will highlight the part of the treatment you are editing by indicating it in red in the top left panel
Click Next
The pictures will not change so click next
The next option is area treated, we have neck selected but this can be edited to indicate: Muscle/Tendon, Bone/Joint or Landmark/Contact
We will change this from Neck to Cervical by clicking the green tab, Bone/Joint
Click the icon named Cervical
Click next
Note the body area treated in the top left panel has now changed to cervical
The patient initial position described as sitting, we will add a secondary position of ‘head neutral’ to further explain the position the patient begins in
Click next, note in the initial position it now has sitting and head neutral directly underneath in the top left panel
We will click next to move through the the wizard for areas we are not changing,
Doctor’s initial position, patient’s contact, Doctor’s contact, Patient’s movement, Doctor’s movement, Patient final
The last button for the wizard, Add precision to your technique enables the user to add options for notation in the treatment.
Observe the bottom right panel has changed, bottom grey area represents what will be seen below the treatment in the player
Click the green tab above Elements
We will add a text box to allow the Doctor to type notes for the treatment by clicking ‘text area’ and dragging it to the grey box below
Click the new element ‘text area’ and dotted black line will surround it
Note when we clicked this the green tab above changed to ‘properties’
Below Common Settings double click in the right column ‘custom text’
Delete ‘custom text’, type Notes, returning after to save the new label
Click close
Observe the new treatment populates the grey area of the ‘My Hands-on’ right pane

Modifying a Modality

Click the top blue tap: modalities
Click Public modalities in the left navigation panel
Select the category of modality you wish to modify: Laser, EMS, Diverse and Disc Compression
We will select Laser by clicking in the left navigation panel
Observe modality icons now populate the grey right panel
Select the modality to edit by right clicking on the modality, click the first laser icon in the top row
Click copy
Click my modality in the left navigation
Right click in the right grey panel
Click paste
Click to select the laser and click edit in the top toolbar
We will change the name to laser neck to indicate more precisely the body area we are addressing
We will not change the description, the interest and the billing code, we also will leave it as laser for type of therapy
Click next to move to the second screen of the wizard
We want to customize this modality for our patients so we will change the pictures associated
To replace the images we will delete the ones currently demonstrating the modality
Click the picture in the grey panel and click remove in the toolbar above
Note a pop up window appears asking you to confirm the removal of the image
Click Yes
Click the upload picture button
Click the blue upload picture button
Browse your computer for the correct images
To upload multiple pictures at once, click the first picture and then hold down the control key and continue to click the additional pictures
Click open
Observe the picture have been uploaded to Docurehab
click save to close the upload box
Click next
We will not change the body area for this, click next
Will add an option for typing notes
Click the green tab elements above the grey panel
Click text area and drag it down to the grey area
Click to select the text area, a dotted black line will surround the element
Observe with the element selected the properties tab is now highlighted
double click where it says custom text and delete custom text
Type notes
Observe there is now a note section and in option
Click close
click save and close
Observe the modified modality now populates the grey right panel displaying the new pictures

Editing Exercises

Click the top blue tap: Exercises
Click Public exercises in the left navigation panel
Select the category of exercise you wish to modify: with no equipment, pertaining to a specific condition, light equipment or heavy equipment
We will select No equipment by clicking in the left navigation panel
Observe there are two categories: stretch and active range of motion
Click Stretch in the left navigation
Observe exercise icons now populate the grey right panel
Select the exercise to edit by right clicking on the exercise, click wrist stretch in the top row
Click copy
Click my exercise in the left navigation
Right click in the right grey panel
Click paste
Click to select the wrist stretch and click edit in the top toolbar
We are not making changes to the first screen of the wizard: we will leave the name, the description, the interest and the billing code, we also will leave it as no equipment
Click next to move to the second screen of the wizard
We want to customize this exercise for our patients so we will change the pictures associated
To replace the images we will delete the ones currently demonstrating the exercise
Click the picture in the grey panel and click remove in the toolbar above
Note a pop up window appears asking you to confirm the removal of the image
Click Yes
Repeat this step for all 5 images
Click the upload picture button
Click the blue upload picture button
Browse your computer for the correct images
To upload multiple pictures at once, click the first picture and then hold down the control key and continue to click the additional pictures
Click open
Observe the picture have been uploaded to Docurehab
click save to close the upload box
Click next
We will not change the body area for this, click next
Will add an option for typing notes

Modifying a Chart

Click the top blue tab, Charting
In left vertical pane click ‘public Charting’
Choose the type of chart we will modify
We will choose a subjective chart by clicking subjective in the left vertical pane
Select the type of subjective chart, we will choose ‘patient health questionnaire’ by clicking in the left panel
We will modify the first questionnaire, HQ1 by right clicking on the chart icon in the grey right pane and clicking copy
Click ‘My charting’ in the left vertical panel
Click the subjective folder under my charting
Right click in the grey right pane and click paste
Click the HQ1 icon in the grey panel and click edit in the toolbar above
This chart is now able to edited
The chart wizard has three steps, we will change the name of this chart to General Medical Background
The chart will still be described the same, it is still a custom form and eligible to be filled out by both Doctor/Therapist and Patient
Click next
We will not change the pictures associated with this chart
Click next
The Charting wizard is now open
In the middle of the left pane are two green tabs: Elements and Properties
Elements indicate the manner information is input and these are broken down by fields, selection and label
Properties indicate the format of the element, example font color, the text used to label the questions and the text used to answer the questions
We will eliminate ‘gums bleed easily’ from this chart by clicking in the grey area on this question, note a dotted black line now surrounds this element
Black dotted lines allow you to know which element is selected and may be edited
We will click delete, note this element has now disappeared
Now we will change options in the current drop down menus included in the chart
Click Diabetes, the current options include NA, was never a problem, was a problem before and is currently a problem
Again note the green properties tab is now highlighted with the edit options listed below in the white space to the left of the grey bottom panel
We will add an option by clicking ‘add option’
Scroll to the bottom and find option 5
Double click in the left column parallel to option 5
Type controlled with medication and hit enter
If you want the answer to be in different font color than the question click on the color picker in the 4th row down in the left column
To maintain consistency we will select blue
We will repeat this for Polio, click Polio
Click add option in the left panel
Scroll to option 5, double click in the left column
Type controlled with medication
Click in the left column of the fourth row to change the font to blue
Observe these drop down menus now have 5 options for the patient to select from
The patient may have additional information they wish to share, we will add a text area for this note
In the left vertical panel click the green tab elements
Click text area and drag it to the right into the grey panel
Click on the new element to select it for editing
Double click in the left column, custom text
Type Patient notes and hit enter
Note there is now an area for the patient to type additional information for the doctor or therapist
Click the close button on the toolbar above
Click save and close
Note in the ‘my charting’ the chart General Medical Background now populates the grey panel

In addition if you want a modified chart to replace a current chart in a current protocol, this substitution will need to be done manually

Creating a New Chart

Click the top blue Charting tab
Click ‘My Charting’
Click new in the top of the right pane
In the top text box type the name for the chart, this will be an Health History
Next add a description: patient health history
Click Select Folder
Observe a pop up window appears with the folders we created to organize the personal charts
We will select subjective by clicking on the it
Click save
To determine where in the notes it will print we will select a Tag
Remember the SOAP notes should print in the order for the acronym Subjective, objective, assessment and plan
Click Tag beneath the description and select the best placement in the print order this would be, this is a subjective form completed by the patient so we will choose patient portal
Click Save
This form is not a billable service, so we will not add a billing code
The type of form is a custom form so that it can be filled in
Determine who the form will be submitted to: the patient or the doctor. In this case, the form will be completed by the patient, click patient
Note that once we have completed these fields the wizard turns the next button from blue to green which means that all fields are complete and we are able to move forward
Click next.
If you would like to add a picture or video click  either ‘Upload picture’ or ‘Upload video’
In the dialog box click the blue upload picture button
Browse in the folder your pictures are stored, select the correct picture and click open
You can select more than one photos by hitting the control key and then selecting both photos
Observe the pictures are uploaded, click save to close the dialog box
Click Next

Creating a Chart Part 2

This screen is to create the content of chart. 
The options on the left side allows us to bring elements to the right side and these elements are then customized
The green tab elements on the left in the left navigation panel represent manner the questions will be asked
The green tab properties on the right in the left navigation panel represent what the question and answer will be as well as the formatting: this includes font color, font size, font type
Observe that the Elements are presents in five different formats
Date field: a calendar module to select dates
Text field: a blank area to type one or two words
Text area: a blank area to type an explanation or information
Drop down menu: selection tool that has the answers in a menu that scrolls
Radio button: A selection tool that has the answers as round buttons to click for the answer, this has three options to it include NA which when selected will tell the system to ignore the entire question when printing
First we will add a title to the chart in order to identify it quickly, labels are creating by clicking the green tab elements in the left navigation panel
Click Text Label and drag it to the middle of the top of the right grey panel
Click the element in the grey to select it, when selected it will have a dotted black line surrounding it
Note when an element is selecting in the right pane the properties tab become highlighted with the modification options below
In the left navigation panel double click in the third row down, in the right column described as value where it says custom text
Delete the words custom text and type Health History and then hit the enter button
Observe the text now reads: Health History

Creating a Chart Part 3

The first question is first name
Click the elements tab, click text field and drag the element to the grey right panel to the next line after the label
Click the element in the grey to select it, when selected it will have a dotted black line surrounding it
In the left navigation panel double click in the third row down, in the right column Delete the words custom text and type first name and then hit the enter button
If you want the answer to print in a different color from the question go to the fifth row down in the right column of the ‘Option’ editor and double click, a small color pick box will appear on the right of the black bar
Click the color pick box and click on the the color you want the answer, we will select blue
We will repeat this step for the last name (Repeat the step)
Click elements, click date field and drag this onto the same line as the first and last name
Click the element to select for editing the preferences
Double click in the custom text column in the left navigation
Delete the words current text and type Today’s Date hit the enter button
If you want the answer to print in a different color from the question go to the fifth row down in the right column of the ‘Option’ editor and double click, a small color pick box will appear on the right of the black bar
Click the color pick box and click on the the color you want the answer, we will select blue
The next question addresses disease issue the patient has had or currently has but first we will create a text label to address the actual question
In the elements select text label and drag to the right grey panel, click to select the element and edit the preferences
Double click in the box custom text and type the question: “Do you have or have you ever had any of the following? and hit enter
Notice the text wraps and is now on four lines, to keep the question as one line we will change the length of the field where the question appears
Double Click in the right column of the row Element width in the left navigation panel 
We will enlarge the label by typing in a larger number, we will use 300 due to the question is longer
You can see that the label is much easier to read
We will use the check box to identify yes answers to this question
If the patient checks the box they have or have had an issue with this disease
Select check box and drag into the right grey panel
Double click in the left column of the left navigation in custom text and we will type alcoholism, hit enter
We will repeat this step, click check box, drag to the grey panel, click to the select the element and double click in custom text to change this to Anemia, hit enter
We have several other diseases to include and to speed up the process we will copy the last element we added, Anemia
Click the element in the grey and click copy located in the toolbar above the grey box
Click paste
Click to select the new element and drag it to where it should be placed, keep this element selected and go to the left navigation and double click in custom text
Delete custom text and type Cancer, hit enter
Observe we have duplicated and edited the element to simplify the process
We are going to skip to the next section of the chart which will be resistance to infection.
Select text label and drag it to the next line
Double click custom text and type: What is your resistance to infections? and then enter
The element width on this will also need to be changed, we will double click in the and delete 150 and make it 300
Now to add the items we will address with resistance to infections we will use the drop down menu
In the left navigation under the elements tab click drop down menu and drag it to the right grey panel where you wish it placed
Select the element by clicking on it and then double click in custom text
Type ‘Catch Colds Easily’ and hit enter
Double click in the right column of option 2 and type Yes and hit enter
We will need to add another option
Click the ‘Add Option’ button, note a new option table was added to the properties scrolling panel
Type No in the blank text field and enter
Note we left NA as option 1, if the patient leaves the NA it will not print on the report
We will change the color for yes and no to blue by clicking the color picker and selecting blue
We will use the copy and paste to add another item to this portion
Click the element in the grey panel, click copy and click paste
Drag the new element to position it correctly
Double click in the properties where it say ‘Catch Colds Easily’
Type ‘Have Frequent Sinus Trouble’ and hit enter
We will change the label width from 100 to 200 to keep the text on one line
Note the options are already saved from the creating the first element
Next we will move on and add a place for the patient to type additional information
In Elements click Text Area and drag to the grey panel
Select the text area and in properties double click in custom text
Type Additional information, again we will need to change the label width
We will change it to 175
To have their note in a different color font, go to font in the properties scroll bar and double click the color picker box and select blue
Click Close
Click save and close
Observe there is a new chart in my charting

Adding a Second User

Click the top blue tab, preferences
In the left navigation panel click user management
Click new in the top toolbar below the blue tabs
Observe a pop up window comes up
Add the email address of the second user they will use for logging into the system
We will use This email address is being protected from spambots. You need JavaScript enabled to view it.
This person is our front desk assistant
We will label her profession as administrative assistant
Click Save
Upon clicking save an email will be sent to this address with a link to finish registration of the account
Click OK
Go to the email address
See the link is sent immediately
Click the link
Observe the new user is redirected to a page to complete setup
Add the first name: we will use docurehab
Add the last name: we will use demonstration
Add the best phone number to use for contact:  we will use 111-222-3333
Create a password: we will use demonstration1
Retype password: demonstration1
Click accept terms and conditions and click sign up
The new user account has been created and activated

Setting Appointment Book Time blocks

Click the top blue tab preferences
Click in the left navigation panel on appointment book
Observe the default types of appointments, each has been assigned a block of time in minutes
To edit the block of time double click on the type of appointment
Observe a pop up box appears “Add new appointment time”
We will change the new encounter from 30 minutes to 45 minutes by either clicking the arrow up
or by deleting 30 and typing 45
Click save
To change the appointment type
Double click on the type, we will select update encounter
Click the ‘edit title’ button in the toolbar above
Delete update encounter and type Re-evaluation and click save
To add a new category of appointment click the ‘New title’ button in the toolbar above
Observe a pop up appears ‘Add new appointment title’
We will add consultation, type consultation
The time will default to 30 minutes
We will adjust this to 20 minutes by clicking the down arrow until it says 20
Go the blue tap appointment book
Click my calendar in the left navigation panel
Select the user you are scheduling for, we will select docudemo
Double click anywhere in the calendar in the right pane
To demonstrate the changes select any patient
Go to purpose of visit, click the arrow
Scroll down to consultation
Observe when we selected this the duration of this type of visit is set for 20 minutes
Click close and observe the calendar now has an appointment with 20 minutes blocked on the calendar

Setting Patient Portal Preferences

Click the top blue tap preferences
In the left navigation panel click patients
Observe there are three action choices for patients
We will set the patient portal so the patient will have their pre-appointment paperwork automatically accessible once they have activated the account
In the last segment click the button select resource
Observe a dialog box appears asking you to select the resource from the above tabs
The automated patient questionnaire is listing in the protocols due to it is several questionnaires grouped together
click the top blue tab protocols
Click Charting in the left navigation panel
Click Doctor portal
Click new patient evaluation
Right click the green folder icon in the right grey panel
Click Select Resource
Observe you are automatically returned to the preferences for the patient
In patient portal section it now says:
Public protocols: charting: Doctor portal: New patient eval: automated new patient eval
We will sign into a new patient account that has been activated
Observe the only green tab reads homework
In the right panel are the icons representing each part of the patient health questionnaire
click play, observe the questionnaire is loaded into the player to allow the patient to complete the forms

Uploading Patients through CSV

To upload Patient Demographic Information through import:
Click the top blue tab preferences
In the left navigation click patients
In the right grey panel click the button ‘download CSV template’
A pop up box appears ‘opening docurehab_patients_sample.csv
and open with microsoft office excel
Double click Ok
Observe an excel document opens
Note the first row is populated and each column corresponds with a field from the patient profile
The next three rows are populated with the information for the sample patients that are present in each new account by default
Each row represents a patient
Each column represents demographic information about the patient
Click the office button in the very top left of the screen of the Excel
Click save as and check the folder the CSV is saved to, most systems store downloaded documents in a temporary file, change this to documents and click save
A dialog box will appear warning you that the file may contain features that are not compatible with CSV. Do you want to keep the workbook in this format, click yes
We will add a new patient to the CSV

Go to the location the CSV is saved and select the file

In the grey panel click upload
A pop up box will appear telling you 1 patient has been uploaded
Click the top blue tab patients
Observe the new patient is included in the patient list
click Jack black
Click Edit
Observe the information for his profile is already populated
